HEALTH INSURANCE AND OTHER COVERAGE
I accept Premera Blue Cross, First Choice, and Group Health Access PPO. I also provide receipts for out of network benefits.

PAYMENT
I accept cash, check or credit card.
Payment is due at the beginning of each session.
